AGE RATES

$60 per month - 5 years to 20 years

$100 per month - 21 years to 59 years

$120 per month - 60+

Onetime $200 enrollment fee for new patients.

$300 couples  and families if enrolled together.

If you drop your membership and wish to re-enroll there will be a $200 non-refundable fee.

 

Our primary services provided

  • Annual physicals, (preventative care): Physical examination, pap smears, sports physicals, ordering routine and diagnostic laboratory tests, discussion of and ordering of preventative tests such as colonoscopies, mammograms, etc.

  • Treatment of acute issues: Including, but not limited to, Upper Respiratory Infections, Urinary problems, Gastrointestinal problems, injuries, etc.

  • Treatment of chronic conditions: For example, Diabetes, Hypertension, COPD, Arthritis, etc.

  • Joint injections: Knee, Hip, Shoulder, Plantar Fascia and Trigger Fingers

  • Incision and Drainage: Abscesses

  • Electrocardiograms (EKG)

  • Nebulizer treatments: For Asthma and COPD Exacerbations

  • Shave, Punch, Excisional biopsies and Cryotherapy

Cancellation & Refund Policy

You can cancel Your membership at any time and the membership will be terminated at the end of the last calendar month paid. There is no cancellation fee or charge.
